Gabii is an out of network vision service provider.

Depending on your Vision Benefits, most prescription related purchases from Gabii can be reimbursed to you by your insurance provider.  Click on standard claim forms below or check with your benefits manager to initiate your reimbursement.  After completing your purchase, fill in the claim form; attach your Gabii receipt and send it in for normal processing.  For additional information please contact your insurance provider or benefit administrator directly.
